The MDH Hospice provides compassionate care for people and their families in the comfort of their own home, nursing home or assisted living facility. Our professional team made up of physicians, nurse practitioner, registered nurses, therapists, social workers, clergy and certified nurse aides provides care allowing quality time focused on what is most important to you.

While hospice enrollment signifies a terminal prognosis, it also provides the most unique and personalized health care support for both the patient and family currently available. Hospice care is focused on CARE and COMFORT over cure of the disease.

The MDH Hospice provides care within a large multi-county area covering all of McDonough County and portions of Adams, Fulton, Hancock, Henderson, Schuyler and Warren counties. View our service area map. A registered nurse is on call 24 hours a day, 7 days a week to answer questions over the phone or provide a professional visit. MDH Hospice provides admission visits on weekends, holidays or after hours.

Our agency is open Monday through Friday from 8:00 a.m. to 4:30 p.m. Our office is located in the Medical Arts building on the MDH campus, just east of the hospital main building.

The MDH Hospice is Recognized as a We Honor Veterans Partner.

The MDH Hospice program has been named a 2023 Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) Honors Elite Award recipient by HEALTHCAREfirst, a leading provider of revenue cycle management services, CAHPS and bereavement surveys, and advanced analytics for hospice organizations.

The MDH Hospice was one of just two programs in the state of Illinois to receive the CAHPS Honors Elite status. To earn that Elite distinction, a hospice program must score above the HEALTHCAREfirst National Performance Score on all 24 of the evaluated questions. In all, eight programs throughout Illinois were named Hospice CAHPS Honors. Throughout the United States there were 61 Hospice programs to earn Elite status.

MDH Hospice accepts death as a normal part of life and provides privacy, dignity and highly specialized care for the patients they serve.
