Medical Records

At McDonough District Hospital we take your private health information very seriously and are committed to keeping health information safe. For your convenience, you may call and place your request with the Health Information Management (HIM) Department directly at (309) 836-1580 or you may print the Authorization For Release of Medical Information Form and present it to the HIM Department. You can also access the Online Medical Request Form to request access to your Protected Health Information (PHI) in the designated record set that we maintain.

Hours and Locations

Our Medical Records Department is open from Monday through Friday from 8:00 a.m. to 4:30 p.m. We are closed on Saturdays and Sundays. The Medical Records department is located on the lower level of McDonough District Hospital, and patients can also stop by the Medical Records window on the first floor (near the main entrance) within the Patient Financial Services office, located next to Admitting.

You have the right to inspect and/or obtain a copy of your PHI in your designated record set from McDonough District Hospital. If you request a copy of your records, McDonough District Hospital may charge a reasonable fee based on the cost of labor and materials to produce the copies.

You have the right to inspect and/or obtain a copy of your PHI in your designated record set from McDonough District Hospital.

Legal Notice

By law, we are not required to agree with your request for access to your PHI, and in certain situations, the law requires us to deny access.

If this is the situation, we will advise you of the reason for the denial. Under certain circumstances, you may be able to request a review of the denial.

Sensitive Information

If information contains sensitive information such as mental health/developmental disability, sexually transmitted diseases and/or alcohol/drug abuse, genetic testing, or HIV/AIDS, please use our third-party authorization form process.
