Frequently Asked Questions Regarding MDH Patient Portal
Where does my health information in the MDH HealtheLife portal come from?
All information in the portal comes from the health records of the McDonough
District Hospital organization: hospital, emergency room, surgery, MDH
Medical Group clinics in Macomb and Bushnell, Sports Medicine and Rehabilitation
clinics in Bushnell and on Lafayette Street in Macomb, Anticoagulation
Clinic, Diabetes Education Center, Radiology, Laboratory, Cardiopulmonary,
Sports Medicine & Rehabilitation Services, MDH Monmouth Clinic, Ear,
Nose and Throat Clinic, OB/GYN Clinic, and MDH Drive-Thru.
No information will be available in the portal for services of Home Health,
Hospice, and outpatient Behavioral Health.
What type of information is available on the MDH portal?
- Information will vary depending on the type of test and treatments during
a hospital stay or tests performed as an outpatient. Examples would include;
personal contact information, such as your address and phone number. An
inpatient hospital visit would include medications, allergies, procedure,
lab tests and results.
- Lab results and inpatient reports from June 6, 2016, along with demographic
information are on the portal. Messaging with your MDH Medical Group provider
is available, as well as appointment requests and drug prescription refill requests.
Will I be able to see information from all my visits or tests over the years?
No, only services performed after June 6, 2016, will be available through
the portal.
When will information be available on the portal?
- Your information will be available on the portal within hours of leaving
the MDH facility.

Your information will NOT be available on the portal for the following services:
- MDH Home Health
- MDH Hospice
- MDH outpatient Behavioral Health
How do I register for the MDH HealtheLife portal?
There are two ways to register and both of them require you to provide
a personal email address.
- Register during the usual check-in process in your provider's office or
in the hospital admitting department.

Contact or stop by the Health Information Management department inside
the Business office on the first floor of MDH (near the main entrance)
to register. Email them at him@mdh.org or call them at
(309) 836-1580 between the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday.
You will need to provide your:
- Full Name
- Date of Birth
- Last 4 digits of your Social Security Number
With any of the above methods, you will receive an email containing a hyperlink
that will take you to the portal. You will have to verify your identity
to complete the registration process.
Can multiple patients register in the portal with my email address?
- Yes, the other patients would have to document their allowance for you
to view their records in the portal. Contact our Health Information Management
department at (309) 836-1580.
- You can view, with a single login, all patient records for which you have
been allowed to view. To achieve this, complete the registration for each
patient utilizing the same combination of email address, username and password.
Is my information safe?
Yes. Access to the portal meets current data security guidelines for healthcare
records. You are the only one who can access your patient records or provide
permission to someone else to access your patient records.
What if I have problems accessing the patient portal?

How old do I have to be to have access to my records?
A patient has to be 18 years or older to access their records.
I care for an elderly parent that does not use a computer or have an email
address, is there a way I can help them access their information?
Yes, a patient of 18 years or older can designate someone else to have
access to their information. This will require assistance from our Health
Information Management department at

Is there a way I can access my children's information?
Yes, a parent or legal guardian of a child, whom is under the age of twelve
years old, can access the child's information. This can be accomplished
during the normal registration process for your child's services. It can
also be accomplished by contacting our Health Information Management department at
(309) 836-1580.
Can I give access to someone else to view my records in the portal?
Yes for those individuals 18 years and older, you can give access to someone
else to view your records in the portal. Please contact our Health Information
Management department at
