Welcome to the MDH Patient Portal

The MDH HealtheLife patient portal is available for patients 18 years of age or older.

The portal includes selected patient data collected at MDH starting June 6, 2016.

To register for the portal, an email address is required.
MDH staff will ask for your email address during registration.
How do I register for the MDH HealtheLife portal? Easy, there are four different ways to get signed up.
- You provide a personal email address during the registration process in your provider's office
- At the hospital's admitting department
- By stopping by the Health Information Management department in the hospital.
- Send an email to HIM@mdh.org requesting to sign up for the Patient Portal.
