Licensed social workers and social service advocates are available to assist patients and their families before, during, and after hospital stays. Services offered include:

• Assessment
• Intervention, support and problem-solving assistance
• Information about financial assistance programs
• Short-term counseling for adjustment to illness
• Community referrals
• Assistance with alternative living arrangements, including nursing home placement
• Discharge & aftercare planning
• Advanced Directives (Living Will and Durable Power of Attorney)
